ComplexCare Solutions Chooses Award-Winning Synapse as its Population Health Management Application

PRLog - Oct. 13, 2014 - ANNAPOLIS, Md. -- New York-based health services provider ComplexCare Solutions (CCS) has chosen Synaptic Advisory Partners to implement its Salesforce.com-powered population health management platform,  SynapseTM—a highly customizable, cloud-based application designed to help care teams gain a full 360-degree view of the patients in their population.   As part of the partnership, Synaptic will work with ComplexCare Solutions to tailor Synapse to meet its scheduling, assessment, care management and billing needs.  The project is set to span three separate phases.

Currently, the project is three weeks into phase one and is focused on ComplexCare Solutions’ scheduling needs.  Once this step is completed, nurses and providers in the ComplexCare Solutions’ network will be able to assess members of the community using mobile devices, tablets and offline windows applications.  Synapse will help track assessments and enable providers to diagnose patients and evaluate their risk levels for other conditions.  It also will allow them to track any issues a patient may have, such as medication compliance or mobility challenges.

With these assessments complete, care management teams will then be able to use Synapse to create patient goals, share them with the entire care team, and apply intervention strategies.  Care providers in the field also will be able to use Synapse  for immediate and accurate billing.
