DES MOINES, Iowa, Sept 23 (Reuters) - Democratic presidential candidate Hillary Clinton unveiled a plan on Wednesday to lower out-of-pocket health costs, including expanded coverage of sick visits to the doctor and tax credits for those with substantial medical bills.
