Cardiologists are faced with many daunting challenges in the management of stable ischemic heart disease (SIHD). Difficult decisions must often be made at various steps of the treatment algorithm—from diagnosis through ultimate management strategy. For example, the evaluation of symptoms and the precision of an obstructive coronary artery disease likelihood in a female patient may be arduous. Moreover, women with angina, evidence of ischemia, but no obstructive coronary artery disease frequently have microvascular dysfunction. When treating angina pharmacologically, patient characteristics and comorbidities, combined with concomitant medications, may obscure prudent individualization of antianginal therapy. Current SIHD guidelines indicate that most patients should have a trial of guideline-directed medical therapy (GDMT) before considering revascularization (in addition to GDMT) to improve symptoms. When revascularization is appropriate, the decision of PCI vs. CABG in patients with multivessel disease is often a complex one, involving a heart team approach, patient preferences, anatomy, other clinical factors, and local resources and expertise. In this program, a panel of experts (co-Chairs Dr. Valentin Fuster and Dr. Karol Watson, and faculty members Dr. Deepak Bhatt, Dr. C. Noel Bairey Merz, and Dr. Benjamin Scirica) will discuss these challenges in lectures, panel discussions, and provocative cases.
